Every church or organization who has adult chaperones must complete, sign and submit a Child Protection Indemnity form as follows:

Only one indemnity form per group is required for each camp stay. For any church or organization who does not have a program in place, we can provide a Child Protection packet that may assist you in establishing such a program.

Churches and organizations may also visit our insurance provider, GuideOne's web site for information at:

    guideonecenter.com
    • select "Preventing Sexual Misconduct Resources"
    • select "Background Checks"
    • select "Background Checks Using ScreenNow"

From GuideOne's website, you will be able to print a free Fact Sheet on background checks. Your church or organization can also find information for the online company "ScreenNow" who performs background checks if you so choose, or you may use another company of your choice for this service.
